Walkers' Medical Consulting will evaluate the entire medical record to ensure the complete and accurate standards as to the medical necessity justifications. The documentation will support the selected level of E/M codes and the accuracy of all CPT, ICD-9-CM, OP, and HCPCS codes that are assigned to each encounter.

All off-site or on-site coded visits and/or corrective actions identified will be in compliance with Medicare qualifying factors under 42 Code of Federal Regulations (CFR) §482.24(c) . These qualifying factors are clear under (CFR) §482.24(c). The medical record must contain information to justify admission and continued hospitalization, support the diagnosis, and describe the patient’s progress and response to medications and services.
